Furthermore, I agree that in signing below, as an owner/agent of my pet, I understand that I am acting on behalf of all owner/agents and I am binding all other owner/agents of any pet I bring to your facility to the requirements needed to utilize the services and the contents of this waiver and release. *

I indemnify and hold harmless Olentangy Premier Pet Care LLC, its successors, heirs or assigns (including the owner, staff and volunteers) and R&KC Adventures II LLC and its successors, heirs or assigns, for any loss, liability, damage, or costs I may incur due to utilizing their services for my pets. *

Our Boarding facility is equipped to provide an environment in which dogs can stay, and have fun while also being as safe and healthy as possible. Olentangy Premier Pet Care is committed to the health and well being of your pet(s) during their stay with us. As an organization, we are committed to creating a place in which our guests have access to our full-time veterinarians should the need arise. While we take every precaution possible to safeguard your pet, injuries and illnesses are possible. Despite all of our efforts, please know that much like kids at school, all dogs at Olentangy Premier Pet Care are susceptible to, among other things, the following: i. Nicks, scrapes, and punctures; ii. Sore or injured joints, muscles, tendons, ligaments & bones; iii. Ingestion of foreign bodies; iv. Fatigue; v. Airborne viruses, common colds and various strains of boarding cough; and vi. Death (especially for animals with pre-existing conditions, etc.) *

Therefore, in utilizing this services and signing below, I understand and agree to the following: All animals stay AT THEIR OWN RISK. Dogs and cats may injure themselves by trying to break through barriers and fencing, and I am responsible for any medical treatment that may be required due to these self-inflicted injuries. OLENTANGY PREMIER PET CARE HAS NO LIABILITY IF SOMETHING SHOULD OCCUR TO MY DOG(S) OR CAT(S) WHILE UTILIZING THEIR SERVICES, as I assume full financial responsibility for injuries that may occur to my animal. I will reimburse Olentangy Premier Pet Care at the time of pickup for any damage to the facility that my pet has caused. *

Please understand that in non-life threatening situations, we will make every effort to contact you, so giving us current and accurate contact information is essential. IN LIFE-OR-DEATH SITUATIONS: I give permission for Olentangy Premier Pet Care to begin life saving care while they attempt to contact me, and I agree to pay for any services utilized during this life or death situation, irrespective of whether I was actually contacted or not. *

I understand and acknowledge that my pet may act differently in a boarding environment and may be prone to aggressive behavior that results in injury (like attacking a cage, etc.) or destructive behavior (bedding, bowls, etc.) that could result in ingestion of foreign materials that could lead to emergency surgery or death. I reaffirm that my pet is at the location at their own risk and I hold harmless Olentangy Premier Pet Care, its employees, successors and or assigns from any liability due to self inflicted injury. Furthermore, I agree that I am solely responsible financially for any treatment that may be required from these self inflicted injuries. *

I agree and understand that any injuries or problems that develop with my dog(s) or cat(s) will be treated as deemed best by Olentangy Premier Pet Care, in their sole discretion, and that I assume full financial responsibility for any and all expenses involved. *

I agree and understand that in admitting my pet(s), Olentangy Premier Pet Care has relied on my representation that my pet(s) is/are in good health and have not harmed or shown aggression or threatening behavior towards any person or any other pet. *

Furthermore, I understand and agree that Olentangy Premier Pet Carehas relied on similar representations by other pet owners, and therefore I indemnify and hold harmless Olentangy Premier Pet Care, its owners, staff and volunteers from any injury from another pet due to misrepresentations made to Olentangy Premier Pet Care. *

I agree and understand that this release, waiver, and indemnity agreement is intended to be as broad and inclusive as permitted by the law of the State of Ohio. If any portion thereof is held invalid, it is agreed and understood that the balance shall continue in full legal force and effect. Should a court determine that any provision waiving liability is deemed unenforceable, Olentangy Premier Pet Care’s liability shall be limited to the funds paid to it by me for taking care of my dog(s). *

When our canine guests participate in their daily daycare activities, part of your dog’s success in our daycare relies on our ability to reward good behavior as well as disagree with unwanted behaviors. Therefore, we use praise as well as very small training treats to teach good doggie manners. If your dog has any food allergies, notify staff immediately upon dropping off. *

I am aware that there is a check in process when dropping off for boarding that can take up to, but not limited, to 10 minutes. *
